Cadence Podiatry (“we”, “us”, “our”) is committed to protecting your personal information in accordance with the New Zealand Privacy Act 2020 and the Health Information Privacy Code 2020.

This Privacy Policy explains what information we collect, how we use it, how we store it, and the rights you have.

As a health care provider, we may collect, use and disclose personal information relating to our clients to provide podiatric care services to them. This may include:

  • personal information such as name, contact details, address, billing information or interaction data.
  • health information such as injury details, medical history, clinical notes or treatment information.
  • unique identifiers assigned by another agency, such as an ACC case number or NHI.
  • website and analytics information such as IP address, browser type, pages viewed, general location information. This technical information is automatically collected when you visit our website and is collected via Google Analytics.

We also collect, use and disclose personal information relating to contractors and suppliers in the performance of our business activities.

2. How we collect information

We collect information about you when you contact us via email or phone with an enquiry or to make an appointment, attend a consultation, are referred to our services by other health professionals or by your employer, interact with our website or provide client feedback.

Overtime we may add online enquiry forms, newsletter sign-ups or online bookings. If these features are added, we will collect the information you provide through those forms.

This Privacy Policy already covers these future features so updates should be minimal.

Most of the information will be collected from clients directly, but there will be some instances when we might need to collect information about you from others such as a family member or referrer. We will only collect information from others when you have consented or authorised that collection, or where collection from others is permitted under the Health Information Privacy Code 2020 or the Privacy Act 2020.

3. Why we collect your information

We collect and use your information to:

  • Provide podiatry assessment and treatment
  • Communicate with you about appointments or enquiries
  • Maintain accurate clinical records
  • Meet legal and clinical obligations
  • Improve our services and website performance
  • Manage billing or administration

We only collect information necessary for these purposes, in relation to this business.
